Introduction

Lipid plays an important role in the body, storing energy and serving as ingredients in biological membranes, steroid hormones, bile acids, and vitamins. The types of lipids mainly include cholesterol (CH), steroids, triglycerides (TAG), fatty acids (FA), bile acids, and so on. Lipid metabolism is the synthesis and degradation of lipids in cells, involving the breakdown or storage of fats and the synthesis of structural and functional lipids. Lipid metabolism mainly includes three pathways: exogenous pathway, endogenous pathway, and reverse cholesterol transport. In the exogenous pathway, after dietary fat is digested and absorbed, TAG and CH combine with proteins and phospholipids to form chylomicrons in the intestinal epithelium. These chylomicrons are absorbed by the lymphatic system, enter the circulation through the thoracic duct, and are transported to the rest of the body and stored in adipose tissue. Endothelium lipoprotein lipase (LPL) cleaves TAGs in chylomicrons to form free fatty acids (FFA) and glycerol. In the endogenous pathway, the liver synthesizes very-low-density lipoprotein (VLDL) and releases it into the systemic circulation. In tissues, LPL cleaves TAGs from VLDL to release FFA, which is taken up by myocytes as energy or stored by adipose tissue. When LPL cleaves TAGs, the concentration of cholesterol in the lipoprotein increases to become a smaller and denser lipoprotein called intermediate density lipoproteins (IDL), and the action of LPL continues to form LDL, which is smaller and denser than IDL. The reverse cholesterol transport pathway involves transporting cholesterol from non-hepatic peripheral tissues back to the liver. This process is primarily regulated by the ATP-binding cassette transporter on HDL, which transfers cholesterol to HDL. However, metabolic disorders in the body can lead to many cardiovascular diseases, and lipid metabolism markers are often used to predict the risk of cardiovascular disease. Commonly used lipid metabolism biomarkers include C4, CETP, 7-ketocholesterol, PCSK9, pre-β1 HDL, etc.
