Introduction

PCSK9 is the ninth member of the proprotein convertase family and is expressed in many tissues and cells. The proprotein convertase are serine protease enzymes that functions in converting an inactive proprotein into an active form by cleavage. The main function of PCSK9 is autocatalysis, which converts proPCSK9 into active PCSK9. PCSK9 plays an important role in lipoprotein metabolism. PCSK9 binds to the epidermal growth factor-like repeat domain of the low density lipoprotein (LDL) receptor (LDLR). Under normal conditions, LDLR not bound to PCSK9 can be recycled to the cell surface, clearing lipoproteins from the circulation, mainly LDL, thereby reducing the concentration of LDL or LALR can be degraded by lysosomes. PCSK9, after binding to LALR through its prosefragment, internalizes with LDLR, induces a conformational change in LALR, resulting in its inability to form a closed conformation, reducing normal circulation of LDLR and increasing LALR lysosomal degradation. Thus expression and release of PCSK9 in the circulation increases the concentration of plasma LDL-C. If PCSK9 is blocked, more LDLR is recycled and present on the cell surface to remove LDL from the extracellular fluid, thus blocking PCSK9 can lower the concentration of blood LDL. In mice lacking PCSK9, the accumulation of cholesteryl esters in the aorta is significantly reduced. Conversely, overexpression of PCSK9 induces excessive atherosclerosis. Therefore, PCSK9 acts as a lipid metabolism maker to detect cholesterol homeostasis.
