The intracellular microenvironment secretes HCl to the resorptive microenvironment via HCO3/Cl exchange, maintaining the pH around 4.5. This acidic environment first mobilizes bone minerals, and then the demineralized organic components of the bone are degraded by cathepsin K. An increase in bone resorption leads to a decrease in bone mass. Bone formation exceeds resorption during childhood, and bone resorption exceeds bone formation with increasing age. Biomarker reflects bone resorption activity from bone release to blood. Therefore, biomarker is generally used to predict bone-related diseases and to detect bone resorption. Bone resorption biomarker mainly includes CTX, DKK-1, DPD, ICTP, NTX, Phosphorus, PYD, RANKL, SOST, and TRACP-5b.
