Introduction

Tartrate-resistant acid phosphatase (TRACP) is a member of the diverse lysosomal enzyme group and has two forms in the circulation: TRACP-5a and TRACP-5b. TRACP-5b is derived from osteoclasts. During bone resorption, osteoclasts secrete TRACP-5b, which produces reactive oxygen species to digest bone degradation products of the bone matrix in the microenvironment. Therefore, it is used as a marker of the activity and numbers of osteoclasts. When TRACP-5b is released into the circulation, it is hydrolyzed by proteases to form fragments that are excreted in the urine and metabolized in the liver. TRACP-5b has several characteristics: i) it is specific for osteoclast number and activity; ii) it is associated with traditional bone resorption collagen markers; iii) it responds rapidly and significantly to anti-resorptive therapy; iv) it has a prognostic value for predicting an increased risk of bone mineral density (BMD) and fracture risk. In addition, TRACP-5b has some advantages in analysis and pre-analysis, exceeding many collagen markers. i) Serum TRACP-5b activity has a lower CVa and CVi because of its small diurnal variation. Compared with NTX and CTX, TRACP-5b is almost unaffected by meals; ii) TRACP-5b is not affected by kidney and liver diseases. Unlike collagen markers, they accumulate at high levels when kidney function or liver function is impaired.
