SOST is a secreted glycoprotein that is mainly expressed by osteocytes as a negative regulator of bone formation. SOST is a marker of mature osteocytes, and its expression gradually increases as osteocytes mature and acquire intact molecular features. Introduction

Sclerostin (SOST) is a 190 aa secreted glycoprotein, which is mainly expressed in bone matrix and cartilage matrix, and can inhibit the mineralization of osteoblasts in cell culture environment. It belongs to the DAN family of bone morphogenetic protein (BMP) antagonist. SOST inhibits osteoblastogenesis and preosteocyte differentiation of osteoblasts. SOST inhibits bone formation primarily by inhibiting the Wnt/β-catenin signaling pathway, and its expression is regulated by mechanical unloading and oestrogen deficiency. The Wnt/β-catanin signaling pathway is critical for the differentiation and proliferation of osteoblasts. Osteocytes secrete SOST, which regulates bone turnover by responding to mechanical unloading or oestrogen deficiency. SOST inhibits bone formation by inhibiting osteogenic differentiation of mesenchymal stem cells or osteoprogenitor cells and proliferation of osteoblasts. At the same time, SOST can also stimulate osteocytes to secrete RANKL to induce osteoclasts formation, thereby increasing bone resorption. In the human body, serum levels of SOST increase with age and are inversely correlated with bone turnover markers. The highly specific expression pattern and unique bone phenotype make SOST an effective target for the treatment of osteoporosis and the promotion of fracture repair.
