Troponin is a complex of three globular contractile regulatory proteins (troponin T, I, C). They are located in regular intervals in striated muscle filaments and inhibit contraction by preventing the interaction of actin and myosin. Compared to other biomarkers, the isoforms of troponin T and I found in skeletal muscle and cardiac muscle are different, so that they can be used as cardiac-specific biomarkers. Troponin complexes play a key role in regulating myocyte contraction. The complex consists of three subunits that regulate actin-myosin interactions: T, I, C. Troponin T binds the complex to tropomyosin wrapped around actin. Troponin I inhibits the actin-myosin interaction. Troponin C binds calcium and thereby regulates the actin-myosin inhibitory effect. In the presence of action potentials, intracellular calcium concentration increases and calcium binds to troponin C, resulting in conformational changes of troponin I and tropomyosin, thereby eliminating the inhibition effect of actin-myosin interactions. This process occurs in skeletal and cardiac muscle, but not in smooth muscle. Troponin C expresses the same isoform in skeletal muscle and cardiac muscle, while troponin T and I are different isoforms in skeletal muscle and cardiac muscle. Since troponin T and I isoforms are specific to the heart muscle, they can be used as cardiac-specific biomarkers.

In many cases, including heart failure, cardiac troponin (cTn) levels can also be elevated, which has important prognostic value. Elevated cTn levels can be found in both acute and chronic heart failure. In acute decompensated heart failure (AHF), elevated cTn levels are associated with increased short- and long-term mortality and increased readmission rates. Studies examining only cTnT in AHF found that the risk of AHF increased with the increase of cTnT levels, and that the increase of cTnT levels (≥0.1ng/ml) was associated with an increased risk of heart failure readmission and death. Elevated cTnT levels at the time of AHF hospitalization and discharge are predictive of worsening heart failure, increased risk of cardiogenic death, and total mortality. Similar to cTnI, cTnT is prognostic for the risk of death and morbidity.
