Introduction

Procollagen type I N-terminal propeptide (PINP) is derived from type I procollagen. Procollagen consists of a triple helical molecule which contains two α1 (I) chains and one α2 (I) chain. The triple helix is flanked by nonhelical telopeptides, N-terminal propeptides (PINP) and C-terminal propeptides (PICP). After cleaved by specific endoproteases, it released type I collagen and circulating PINP and PICP. The circulating PINP concentration is proportional to the amount of new collagen matrix being formed and subsequently new mineralized bone. Therefore, PINP is a serum biomarker associated with bone formation. PINP has a high sensitivity, furthermore, it is less sensitive to variations during the day and after food intake. Therefore, PINP concentration is a sensitive biomarker to monitor therapy in Paget's disease, osteoporosis, van Buchem disease, rheumatoid arthritis, and so on. Trimeric PINP is a good marker for follow up of disease activity in mono-ostotic disease or in case of abnormal kidney function. Especially in osteoporosis, PINP is the marker of choice to monitor therapy. In addition, studies in rat have shown that PINP as a bone formation biomarker in rodents, could be a useful tool for preclinical evaluation of treatments.
