Introduction

OPG is an inhibitory factor of osteoclastogenesis, which is also known as a member of the tumor necrosis factor receptor superfamily (TNFRS), it is mainly secreted by osteoblast lineage cells, vascular endothelial cells, B cells and the epithelial cells of gastrointestinal tract, lung, breast and skin. OPG is a glycoprotein which can exist as a 60 kDa monomer or a 120 kDa dimer in soluble form. The monomer of OPG can be linked by disulfide bonds to form the dimer that plays a critical role in bone remodeling process as an activity factor. The dimerization of OPG can increase its affinity for RANKL, while OPG in the form of a monomeric protein cannot inhibit the interaction of RANK-RANKL by competitive binding with RANKL, which is due to its insufficient affinity with RANKL, for these reasons, OPG dimerization is essential for its inhibition of RANK-RANKL. The RANKL/RANK/OPG signaling pathway is discovered in 1990s, RANK is a member of TNFR that constitutively expressed in prostate, liver, osteoclast and other tissue cells, while RANKL is usually expressed in osteoblasts, stromal cells and T cells, OPG act as a decoy receptor to RANKL. RANKL can be bind to RANK that expressed on the surface of preosteoclast to promote the differentiation and maturation of osteoclast, however, OPG blocks the activation of RANKL by binding and neutralizing RNAKL, and results in the inhibition of osteoclastogenesis and promote the apoptosis of osteoclasts. Due to the critical role OPG played in bone remodeling, it can be considered as a biomarker for several bone diseases. For instance, OPG overexpression is related to osteopetrosis as its inhibition function to osteoclast production, on the contrary, OPG deletion is related to osteoporosis as its enhancing for bone remodeling. In addition, the expression level of OPG is higher in patient suffered with cancer, and OPG has the role of promoting angiogenesis, bone metastasis and multiple myeloma. Expression of OPG therefore is coordinated with bone metabolism and regulates bone density with RANK and RANKL.
