Assay Background

HE4, also known as WAP four-disulfide core domain protein 2, is a reliable biomarker for ovarian cancer other than CA-125. HE4 is a glycoprotein belonging to the whey acidic four-disulfide core protein family, and can be used as a diagnostic biomarker for malignant ovarian tumors. HE4 has two WAP domains. Generally, proteins containing WAP domains are small secreted molecules, secreted by pro-inflammatory cells, have serine protease inhibitory activity, and participate in natural defenses against microorganisms. High levels of HE4 mRNA can be detected in the lungs, kidneys, and trachea. In addition, its mRNA can also be detected in the prostate, breast, and endometrium, while HE4 expression is rarely detected in the ovary. In epithelial ovarian cancer, the expression of HE4 is up-regulated, so scientists believe that detecting the serum concentration of HE4 can help the diagnosis of ovarian cancer. CA-125 is a commonly used biomarker for ovarian cancer, but its sensitivity and specificity for ovarian cancer are not high. The detection of HE4 can supplement this, thereby significantly improving the sensitivity of CA-125.
