CK-18

CK-18 is the main intermediate filament protein in the cytoskeleton, and is an intracellular protein with high expression in a variety of epithelial cells. CK-18 participates in the cell death pathway and is an important biomarker for evaluating cell death. Most intracellular CK-18 is insoluble. During cell death, it can be released into the blood as the cell ruptures. During cell apoptosis, CK-18 is catalyzed by caspase to undergo hydrolysis. Caspase cleaves CK-18 into three fragments at two sites and releases the resulting fragments into the blood. Therefore, detecting the concentration of the caspase-cleaved fragments of CK18 in the blood can reflect the degree of apoptosis. The M30 assay is based on this principle using a monoclonal antibody that can recognize the neoepitope Asp396 of CK-18. The CK-18 M65 analysis method is used to assess total cell death. The combination of M65 and M30 assays can monitor the proportion of apoptosis in total cell death and evaluate the situation of cell necrosis. Apoptosis increases in patients with sepsis, and this may cause multiple organ failure and even death in patients. Therefore, CK-18 as a biomarker that can be used to monitor apoptosis has also been used in the study of sepsis.
