Type II collagen is the major structural protein of cartilage, accounting for about 50% of the extracellular matrix. Studies have shown that fragments derived from collagen degradation are potential markers for remodeling cartilage pathologies. Among various type II collagen degradation products, a neoepitope (Collagen Type II Cleavage, C2C) produced by intra-helix cleavage of collagenase has been well studied in vivo. Introduction

The articular cartilage matrix is mainly composed of type II collagen and proteoglycans such as aggrecan. Collagen fibrils provide tensile strength to maintain tissue integrity, while aggrecan interweaves with collagen fibrils to increase compression stiffness. Type II collagen is important for endochondral bone formation, growth, and normal joint function. Over-cleavage of type II collagen by collagenase is a critical step leading to destruction of the cartilage matrix. Articular cartilage matrix destruction is a feature of osteoarthritis (OA) that may be associated with unbearable pain and daily life difficulties such as walking, up and down stairs, and other movements that accompany knee flexion or stretching. Once a radiographic change occurs, the development of cartilage damage cannot be prevented. Therefore, in order to prevent the development of OA, early cartilage damage must be detected. Collagenase-cleaved type II collagen neoepitepe (C2C) is a biomarker that responds to type II collagen lysis in patients with anterior cruciate ligament (ACL) injury or OA. Neoepitope C2C located at the C-terminus of the 3/4 length fragment, is 10 amino acids long [GGEGPP(OH)GPQG]. This neoepitope originally designated COL2-3/4Clong.
